Abstract

|
Anisocoria (inequality of pupil size between two eyes) is one of the common signs in neuro-ophthalmology. Although often benign, it can also represent the sole sign of a life-threatening disease. The parasympathetic system constricts the iris through sphincter pupillae muscle contraction, while sympathetic system dilates the iris through dilator pupillae muscle contraction. Asymmetry in the pupillomotor neural output or their muscular forces results in anisocoria. The diagnostic approach to anisocoria has remained relatively unchanged over time. However, there have been several notable changes to the list of possible causes, trends in diagnostic techniques, and management. The modified algorithm of anisocoria is illustrated in this review.
